[임산부 정보]미세먼지, 임신부 및 태아에 어떤 영향을 미칠까?

최근 국제학술지인 Hypertension(고혈압)에 미국의 보스톤지역에서 임신중 노출된 초미세먼지(PM2.5)가 다음 세대인 자녀의 혈압에 미치는 영향에 관하여 발표되어 우리나라 임신부들에게도 시사하는 바가 있어서 초록을 정리해본다.

임신 중 직경 2.5㎛ 이하 초미세먼지 노출과 소아기 고혈압의 위험성

Abstract

대기오염 노출은 성인과 어린이들의 고혈압 위험도 증가와 연관 있다.
최근의 증거는 임신 중 대기오염 노출은 다음 세대 위험을 암시할 수 있다는 것을 보여준다; 그러나 이 상관관계를 조사한 연구는 거의 없었다. 우리는 1998년~2012년에 등록된 보스턴 출생 코호트에서 1,293명의 임신부와 3~9세 사이에 후속 방문한 그들의 아이들에 대한 연구를 진행하였다.

임신 중 1차 노출에 대해 임신부의 거주지 주소와 미국환경보호청(US Environmental Protection Agency)의 대기질 모니터 결과를 연결시켜 PM2.5 초미세먼지 농도를 평가하였다. 우리는 미국 기준 (4차 보고서)에 따라 1차 결과 아이들의 수축기혈압 백분위수를 정의하였으며, 고혈압을 수축기혈압의 90번째 퍼센타일이상으로 분류하였다. 우리의 다변량분석(multivariable-adjusted cubic spline)결과는 임신 말기(임신 26주 이후)에 13 μg/m3 농도 이상의 PM2.5의 미세먼지에 노출된 경우 수축기혈압 백분위수와 혈압 상승의 위험도가 급격히 증가하는 것을 보여준다.


임신 말기 PM2.5 노출의 가장 높은 부분 대 가장 낮은 부분 삼분위는 4.85퍼센타일의 어린이 수축기혈압의 증가 또는 어린이 혈압 상승 위험도가 1.61배 높은 것과 관련 있었다. 임신 말기의 PM2.5의 5 μg/m3 증가는 어린이 수축기 혈압의 3.49퍼센타일 증가 또는 어린이 혈압 상승 위험도가 1.47배 높은 것과 관련 있었다.

우리의 연구결과에 따르면 임신 말기의 PM2.5 초미세먼지노출은 3-9세의 어린이의 혈압 상승과 연관이 있다.

Maternal Exposure to Ambient Particulate Matter ≤2.5 µm During Pregnancy and the Risk for High Blood Pressure in Childhood.

Abstract
Exposure to ambient air pollution has been associated with greater risk of elevated blood pressure (BP) in adults and children. Recent evidence suggests that air pollution exposure in pregnancy may also portend increased risk for the next generation; however, few studies have examined this relationship.

We conducted a prospective study of 1293 mothers in the Boston Birth Cohort (enrolled 1998-2012) and their children who had follow-up visits between 3 and 9 years of age and complete exposure and outcome data. Our primary exposure, ambient particulate matter ≤2.5 µm (PM2.5) concentration during pregnancy, was estimated by matching mother’s residential address to the US Environmental Protection Agency’s air quality monitors.

We defined our primary outcome child systolic BP (SBP) percentile according to US reference (Fourth Report) and classified elevated BP as SBP ≥90th percentile. Our multivariable-adjusted cubic spline showed a sharp increase in offspring SBP percentile and risk for elevated BP when third-trimester PM2.5 concentration was ≥13 μg/m3 The highest versus lowest tertile of third-trimester PM2.5 exposure was associated with a 4.85 (95% confidence interval: 1.38-8.37) percentile increase in child SBP or a 1.61 (95% confidence interval: 1.13-2.30) times higher risk of child elevated BP. A 5-μg/m3 increment in PM2.5 during the third trimester was associated with a 3.49 (95% confidence interval: 0.71-6.26) percentile increase in child SBP or a 1.47 (95% confidence interval: 1.17-1.85) times higher risk of elevated BP. Our findings suggest that exposure to ambient PM2.5 during the third trimester of pregnancy is associated with elevated BP in children, ages 3 to 9 years.
